It’s exciting to introduce new foster children to the family. You may find yourself getting giddy about connecting your biologically extended family to your new foster children, but your foster children have a different experience. Foster children don’t necessarily understand how they fit into the parade of individuals who suddenly take their places in your living room. Foster children, though abused or neglected, still see themselves as part of their birth family unless they’ve been in your home for several years. Transitioning from one family isn’t about taking up residency in a new home. It’s about changing one’s identity, grieving, and finding a way to accept things the way they actually are. When you introduce your foster children to grandparents, aunts, uncles, or brothers and sisters, you need to prepare both your relatives and foster children for the gray areas that exist with these new relationships.

Foster children will often have a set of grandparents already when they show up to your home.

They probably have aunts, uncles, and even perhaps brothers and sisters of their own. Unfortunately, your foster child’s family is probably not in any way intact. Seeing and meeting family members who havehealthy relationships can be upsetting in a way for foster children who feel alienated from their own families.

It’s important to educate your foster child about the people you plan to introduce her to. Telling your foster child stories about these people can help cement the information about these relatives into your foster child’s brain in a way that is meaningful.

I tried numerous times to educate my own parents about the nature of foster care and how foster children experience their new family environment. I provided books and short lectures on foster parenting to try to get my parents to comprehend the situation these foster children were in when they were placed in our home. But my parents, refused to understand and instead desperately wanted these foster children to function as full-fledged grandchildren. Though my parents could have treated our foster children as grandchildren, they needed to respect the process of foster parenting for what it actually is. Foster children often do go home. And foster children need to be able to talk about their birth family without judgment from the foster family. Foster children can come into your home from different races and religious backgrounds and you need to attempt to prepare your biological family for this fact.

Though both your foster children and your extended family may misinterpret the new relationship that can be forged through your efforts as a foster parent, you can still provide ample opportunities for your family to intermingle successfully with foster children. If you enjoy entertaining it might be easier than if you hate putting on family events. Figure out a family situation that is amenable to gathering together the diverse members of your family in a setting that allows everyone to mingle a bit and get to know each other in an informal sort of way. Perhaps a visit to Chuck E. Cheese is in order. An environment like that can help people find ways to hang out together without making a big commitment to conversation that could go awry.

It may be best to introduce your foster children to the people in your family one at a time in an informal way before subjecting your foster child to a big family event like the holidays. If your foster child doesn’t take up residence in your home until the holiday season, this may not be possible, but it can be stressful for a foster child to have to meet a multitude of family members at a holiday event for the very first time.

As your child’s foster parent, it’s up to you to help your foster child and your relatives make sense out of the new relationships that they can have with each other. You can provide an important link that will help both parties understand each other better from the beginning. Give your relatives the chance to interact with your foster children one-to-one, as well as in a group. Foster children will start to feel more like a member of your ?family once he or she starts making relationships with extended family. Knowing the people who are your parents, brothers, and sisters can help your foster child understand you better and enjoy your family environment even more.

LOS ANGELES (Reuters) ? NBC’s new musical drama “Smash” could hardly have a stronger pedigree: Idea by Steven Spielberg, songs by award-winning Broadway producers, stars including Anjelica Huston and Debra Messing, and screen icon Marilyn Monroe holding the whole show-within-a show together.

Yet success couldn’t be harder to assess in the high stakes prime-time TV gamble for NBC as the struggling network looks for its first scripted series hit since 2006.

“You can’t pin a network’s fate on any one show. But having said that, if you ever could, it would be this network and this show,” said James Hibberd, senior staff writer at Entertainment Weekly.

“Smash”, which makes its debut on Monday, is a backstage tale of writers, directors, producers and aspiring actresses working to create a new Broadway musical based on the life of Monroe.

The idea came from Spielberg, and the show is being written by playwright Theresa Rebeck (“Seminar”). It features both cover versions of hit songs and original material written by “Hairspray” Tony winners Marc Shaiman and Scott Wittman.

TV viewers have seen nothing quite like “Smash” in recent years. Show-stopping musical numbers are interspersed with darker casting couch moments and theatrical back-stabbing, giving industry watchers little to compare it with.

“It’s the type of show that could either be a huge hit, or just go down in flames. It’s a little bit different,” said Brad Adgate, senior vice president of research at advertising firm Horizon Media.

“GLEE” FOR KIDS. “SMASH” FOR ADULTS

While “Glee” — the quirky musical comedy about singing high school kids that gave Fox a surprise worldwide hit three years ago — opened the door for more TV musicals, “Smash” is a drama, set in an adult world.

“‘Glee’ caught lightning in a bottle and was so smart to target the ‘American Idol’ and ‘High School Musical’ audience by setting it in a high school. Putting ‘Smash’ in a more grown-up landscape asks whether you can deliver on this genre in an older setting,” said Hibberd.

Spielberg, who producers say has been very closely involved in the script and editing, believes there is enough tension in the creative process to make “Smash” interesting to all-comers.

“I think audiences will be able to relate to (it) whether or not they ever had seen a Broadway show because this is really about the drama of the characters,” the Oscar-winning director told TV reporters in a January video message.

NBC, which has been mired for years at the bottom of the four biggest U.S. networks, is leaving nothing to chance.

The “Smash” pilot was screened in 10 major cities and on American Airlines flights in January. The network has been streaming the first episode on its website and on Hulu, and made it available to download on iTunes, PlayStation and other platforms three weeks before Monday’s official premiere.

The network is desperately in need of a hit after what entertainment chief Robert Greenblatt admitted last month was “a really bad fall”. Pricey new dramas “The Playboy Club,” “Prime Suspect” and comedy “Free Agents” were swiftly cancelled, and audiences in the 18-49 demographic prized by advertisers fell 11 percent.

“‘Smash’ is going to be very important to us, but I don’t believe it is a make or break show,” Greenblatt told TV reporters in January. If it doesn’t work, “it’s not like we are going to go into receivership.”

Adgate said a hit for “Smash” would be “a nice stepping stone” for NBC, which will be lucky to have a show in the Top 20 once football ends its season with this weekend’s Super Bowl.

“Some of their hit shows are starting to fray at the edges. ‘The Office’ is down a bit, ‘Law & Order’ and ‘The Biggest Loser’ have all been down this year,” he said.

“They really have to bolster their programming with at least some shows that can build appointment viewing, and be used as a platform to promote their line-up. I think ‘Smash’ is a show along with singing talent show ‘The Voice’ that NBC is really counting on to turn the ship around this quarter,” Adgate said.

Media Misdirect: Donald Trump Endorses Romney for President (ContributorNetwork)

COMMENTARY | In what was either a political double-cross or a media misdirect of some sort, business mogul Donald Trump announced Thursday that he was endorsing Mitt Romney for president. Much of the media, including outlets like the Associated Press, had been reporting that sources from within the Gingrich campaign had indicated that the celebrity businessman would soon announce an endorsement of their candidate.

Talk about getting juked out of your shoes…

At an impromptu news conference in Las Vegas, as reported by CBS News, Trump told the gathering of reporters, ” Mitt is tough, he’s smart, he’s sharp, he’s not going to allow bad things to continue to happen to this country that we all love…So Governor Romney, go out and get ‘em, you can do it.”

He further noted that Romney seemed to be the only candidate to notice how the country was getting “ripped off” by those that it dealt with and how Romney recognized how OPEC (Organization of the Petroleum Exporting Countries) and China were ” absolutely destroying this country.”

But what about supporting Newt Gingrich? When asked about reports that he was to throw his support behind the former Speaker, Trump said, “They put that out.” He insisted that he never told anyone he was backing Gingrich.

But what about the sources? The National Journal noted that they had confirmed the Gingrich endorsement through Gingrich sources that wish to remain anonymous.

Good thing, considering that their employer has been left without a Trump endorsement.

The Wall Street Journal also reported that sources within the Gingrich camp had confirmed the Trump endorsement.

The Associated Press also ran with the Gingrich endorsement story Wednesday evening. Up until Trump’s press conference, many thought Gingrich had the businessman’s endorsement sewn up.

The good news for Republicans might not be in the endorsement itself, but in the Trump announcement that if Romney went on to win the Republican nomination in August, he would not run as a third party candidate. An independent candidacy, at least from the Republican Party perspective, could spell certain defeat in November, drawing away conservative voters that would most likely vote for the Republican candidate if the independent contender had not entered the race. Of course, leaving the spectre of a Trump candidacy in the cards should Gingrich get the nomination might also sway some undecided voters away from Gingrich as well.

So which was it? Just a bunch of reporters getting their stories wrong or someone misdirecting the media, such as Trump himself (known for manipulating the media and finding ways to keep himself in the spotlight, especially when his television show, “Celebrity Apprentice,” is due to begin a seasonal run). Could there have been a political double-cross, one where certain things were said, perhaps promised, then removed from consideration?

Many scenarios are possible, but it is difficult to see campaign staffers and/or officials within the Gingrich organization confirming endorsement stories without some solid reason for doing so. Politics being what they are, politicians and the people that work for them are usually very circumspect when confirming endorsements and do not base their “leaked” information, even if given anonymously, on wishful thinking.

And a complete media misdirect with an endorsement made in the City of Sin, in the land of big deals and bigger gambles, seems all about presentation and not at all about substance.

Something that has been said about Donald Trump all too often.

Besides, the new season of “Celebrity Apprentice” begins in two weeks.

Oscar is one of several cats who live at Steere House nursing home. All of these cats provide companionship and love for the residents, but only Oscar has the special talent of being able to sense when people are nearing the end of their lives. The nurses were the first ones to figure it out as they noticed how frequently he showed up just at the right time.

No one knows how he does it, but when he detects that someone is near dying, he takes up residence on their bed and usually stays until the funeral director comes to collect the body. During this time, he also offers comfort to the family who are there to be with their loved one during this transition. When there?s no one to sit with the patient, Oscar maintains a solitary vigil. No one dies alone on Oscar?s watch.

People who love their pets probably won?t question Oscar?s abilities, but one of the doctors who works there was a bit of a skeptic. This book is the result of his interviews with family members and staff who shared their experiences with him. Over and over they told Dr. Doza how much the gift of Oscar?s presence had meant to them during a very difficult time. Most people who have cats know the comfort they can bring when they curl up next to you in bed and share their warmth. It?s as if Oscar?s being there normalizes the events and removes some of the fears.

All of the patients on Oscar?s floor are in the final stages of dementia, usually due to Alzheimer?s. Experience and research have shown that two things are often able to break through the haze that envelops them ? music and animals. In the process of telling Oscar?s story, Dr. Doza also gives us insight into this very scary disease. If raising a child is about watching them learn skills, living with an Alzheimer?s patient is the opposite ? they are slowly unlearning them. Each loss is a form of good-bye. While this book doesn?t make the disease any less scary, it does offer comfort and hope for those affected by it.

We may never know just how it works ? how Oscars knows just the right time to show up. Maybe all we really need to know is just that he does.

OK so here is this book with this cool cat on the cover, and you think there is something neat about the whole concept. Maybe you have already heard there is this cat that knows when people are going to die. Well, it?s way more than that. This book, written by a doctor who is not actually a cat person, is more of a tribute to those creatures, human as well as feline, who allow advanced dementia patients to die with dignity.

I imagine that Steere House will not be lacking for residents after this moving depiction. Needless to say, it is heart-wrenching for any family member to place his/her loved one in a nursing home, probably more so when the loved one has dementia. What a gift to know that Steere House exists, where the staff is compassionate, even loving, and treats their residents like family. Where a cat moved in while the building was still under construction, and the management took it as a sign that animals were meant to live there along with the patients. Personally, I find dementia to be a pretty scary topic and generally try not to think about it. The author is a geriatrician who makes it real, even if still mysterious. He interviews family members who speak courageously and honestly about losing their loved ones, and how it helped to have Oscar there at the end.

I learned that hospice is not just for the very end of life, and it is about much more than medical care.

I learned that people who refuse to eat at the natural end of their lives are not starving themselves.

I learned that there is a lot we don?t know about dementia, but we are learning more all the time.

Dr. Sosa writes in a very easy, straightforward style. His patients and their families are very lucky people.

I can?t recommend this book highly enough. It made me laugh and, yes, cry, but mostly it just made me feel better in general. Losing a loved one to dementia is about the most horrible experience one can contemplate, but afer reading this book I feel like I could cope. And Oscar is a pretty amazing cat too.

Good Book, Misleading Title3
I loved geriatrician David Dosa?s 2007 essay in the New England Journal of Medicine ? about Oscar the cat, who by then had seemingly predicted, within hours, the impending deaths of dozens of residents on the dementia unit of a Rhode Island nursing home. He?d been dubbed the ?grim reap-purr? and I was thrilled to see MAKING THE ROUNDS WITH OSCAR: THE EXTRAORDINARY GIFT OF AN ORDINARY CAT and, from that title, eager to read what promised to be an expansion of the essay. So first, to be clear: this book is not much about the cat.

In fact, there might be a mere cumulative total of 20 pages about Oscar. Rather, the book is one part memoir of the doctor and his geriatric practice; one part profile of the dementia unit?s charge nurse; and eight parts profiles of the residents and their families, with a dollop about the end-of-life comfort provided to them by Oscar. Nor does Dosa explore (beyond a couple sentences) the source of Oscar?s instinct ? the theories and research about the physiology of dying and animals? amazing sense abilities.

That said, I?m going to take a sharp turn and say that I liked the book it actually *is*, and that it?s an important book for the elderly and (especially) their caregivers to read. Dosa is frank about the fear, denial, frustration and guilt inherent in caregiving generally, and specifically in losing a loved one in ?the long goodbye? of dementia. He touches on the inadequacies of doctors and the healthcare system and the importance of realistic end-of-life directives. And there are takeaways: that simple diversion is more effective than trying to reign someone in from their altered reality; that it?s important to interact according to who the person is now (in dementia) rather than who they were; and that it?s most important to simply ?be there? rather than necessarily interacting at all. Recommended.

Dosa, a geriatrician with a strong aversion to cats, tells the endearing story of Oscar the cat, the aloof resident at a nursing home who only spends time with people who are about to die. New European pill works against uterine fibroids

NEW YORK (AP) ? New research offers hope for the first pill to treat a common problem in young women: fibroids in the uterus. The growths can cause pain, heavy bleeding and fertility problems, and they are the leading cause of hysterectomies.

In two studies, a lower dose of a “morning after” contraceptive pill stopped the bleeding and shrank the fibroids. It worked as well as shots of a hormone-blocking drug that has unpleasant side effects.

“This is very, very good news. The results are better than we expected,” said research leader Dr. Jacques Donnez, of Saint-Luc hospital at the Catholic University of Louvain in Brussels.

He’s now testing intermittent long-term use of the pill to see if that could help women avoid surgery.

The pill is called Esmya, and it is awaiting marketing approval in Europe. It’s a low-dose version of an emergency birth control pill called ella that came on the market in the United States about a year ago. The new fibroid pill still needs to be tested in the U.S. and won’t be available anytime soon.

Fibroids are benign growths in the uterus that are common in women during their childbearing years, mostly in their late 30s and 40s. They usually go away after menopause. Treating fibroids isn’t easy. Removing the uterus is the only cure; other treatments include surgery to remove them or procedures to shrink them with ultrasound or pellets that cut off their blood supply.

With the discovery that the hormone progesterone, as well as estrogen, promotes fibroid growth, scientists have been looking at a class of drugs that can block progesterone’s effect on the uterus.

Donnez and his colleagues in several European countries tested Esmya, made by Swiss-based PregLem. Their findings are in Thursday’s New England Journal of Medicine.

The two studies involved about 550 premenopausal women whose fibroid symptoms were serious enough that surgery was planned.

One study compared two doses of Esmya with a dummy pill for three months. The second tested Esmya against a monthly hormone-blocking shot that shrinks fibroids but causes hot flashes and, with long-term use, can thin bones.. Women in that study got a daily Esmya pill and a dummy shot each month, or a hormone shot and a dummy pill.

In both studies, Esmya stopped the bleeding and shrank fibroids in most patients and worked as well as the shot, but with fewer side effects. Menstrual bleeding was controlled in over 90 percent of the women on Esmya ? many within a week, compared to 19 percent of those who took a dummy pill.

At the end of the three months, only about half of the participants went ahead with any kind of fibroid surgery. That allowed the researchers to observe whether improvements lasted over the next six months. They did for many of the Esmya patients, while fibroids started growing after a month in the group that got the hormone shot.

Donnez is now studying whether Esmya could be used long-term, given periodically if symptoms return, until menopause, when fibroids usually disappear. That means some women, depending on their age, might avoid having surgery at all, said Donnez. He does between six and 10 hysterectomies a week for fibroids.

Despite newer, less invasive alternatives, the rate of hysterectomies remains high, Dr. Elizabeth Stewart, a professor of obstetrics and gynecology at Mayo Clinic, wrote in an editorial in the journal. There’s a need for good medical treatments and the new research “represents an important step in that direction.”

“It’s amazing to me that so many women have uterine fibroids and yet the treatments we have available are pretty few and far between,” she said.

The new pill is awaiting final European approval as a treatment before surgery, following a recommendation from the European Medicines Agency in December.

In the U.S. and Canada, Esmya will be developed by Watson Pharmaceuticals Inc., which also sells prescription ella, the contraception pill that helps prevent pregnancy for up to five days after sex.

Watson spokesman Charlie Mayr said the company will soon start a study of the fibroids pill in the United States, but it will be several years before it is ready for government review. It will seek approval in Canada early this year, he said.

Drugmaker PregLem paid for the latest studies. The researchers included company employees; Donnez and others have been on its scientific advisory board.
